Payment in full must be received by June 9, 2024. Failure to comply can result in the complete cancellation of your registration. No refunds will be given for any absences, lateness, or cancellations.
I will be held responsible for any and all damages incurred by my child(ren).
By signing this application, I hereby authorize Lev Torah Summer Program to take my child(ren) off camp grounds for trips as part of the camping program.
In the event of a medical emergency, I authorize Lev Torah Summer Program staff to obtain necessary emergency medical treatment for my child(ren). Lev Torah Summer Camp and their staff shall be held harmless from any and all liabilities arising from such emergencies.
It is understood that in the final disposition of any emergency case, the judgment of the camp authority will prevail. The recommendations of the parent will be respected as far as possible.
I give permission for my child(ren) to participate in swimming and all activities held at camp, and to go on all trips. In case of an emergency, I know the camp will make every effort to contact me or any person whom I designate. If no one can be reached, I authorize the camp to provide medical assistance for my child. I release the camp, its directors, and any staff members from any claims of liability that may arise. I allow the camp to send me text message notifications. I agree not to take legal action against the camp. All items left in camp after the summer will be considered camp property.
